I had a similar situation to the woman whose insurance didn't pay for the ambulance because it wasn't pre-approved! The excuse they gave me was they wouldn't pay because I was admitted to the hospital! They even charged $12.00 for transportation, and I live less than a mile away! Disgusting!!
