The 24 Hour Report Form is designed to share information between shifts regarding patients' status throughout the day. It is a very informative communication tool for nurses to utilize and effectively care for all residents. With the new 24 hour report form, it is very difficult and time consuming to communicate and understand the information that is being endorsed between shifts. When a Physician or Health Care Provider calls to follow up on a patient's status it is difficult to obtain the information from the new 24 Hour Report Form, as opposed to the original 24 Hour Report Form, where the information was easily obtainable upon the MD's request. This makes it easier for nurses and Physicians to communicate and other members of the interdiplinary team as well.
