recipient: want it to go to luzerne county or do i send it to parole in harrisburg mycase number is 0003232 of 2006 i sprnt seven years in jail and 2years 2 months on the street i have 2 years 10 months left on parole and i live in Bethlehem but my wife lives in luz

I have completed all the course they put before me finished CADA counselor for drug and alcohol finished lehigh drug and alcohol paid my supervision fee paid court cost the halfway house took the money. not all has been paid but some of it has, i have lived within the law and have not violated any rule why can't i go back to my wife Their is an applicatiuon to terminate probation it is form A-15 i do not know who to addres this to
