PETITION TO SAVE OUR MOLOKAI COMMUNITY HEALTH CENTER

I give my consent to use my signature and my information, for the sole purpose of the removal of the Executive Board of the Molokai Community Health Center (MCHC), and its CEO.

A vote has already taken place of the Executive Board and CEO's removal on September 26, 2019, and the Board continues to display gross negligence by ignoring the Community's request.

Many concerns and issues have arisen at the MCHC regarding access to services, proper staffing and financial management that ultimately lead to the shutdown of the Medical Department for 4 days in August.

I. The Community's Medical care has been compromised.
II. Patient Rights have been violated.
III. HRSA Chapter 6: Accessible locations and hours of Operation: Continuity of Service; The required primary health services of the health center must be available and accessible in the catchments (service) area of the center promptly, as appropriate, and in a manner which ensures continuity of service to the residents of the centers catchment area.
