Bill Before Payment (BBP)

A cable company denied cable service to everyone in our household stating I owed them over $640 for service over 4 years ago in a state I never lived in and never authorized anyone to open an account in my name.  They said I need a subpoena to get a copy of the bill.  The following have been contacted for help: Federal Trade Commission, Federal Communication Commission, Elected Official, Consumer Protection Agency, IRS, and others. 

I want to establish a law / policy that a COMPANY that says you owe a bill MUST provide you with a copy of the bill, free of charge, within 7 calendar days, that at least provides the name, address of the person who got the service or product, account number, date of service / sale, place of service / sale, and amount due?

What if you had moved with a loved one with a major illness closer to the treatment facility and the person with the illness just wants to watch their favorite show on cable or communice online with loved ones? Under the current situation they can be denied even though they did'not do anything wrong.
