To Request Change of My Last Name

My name is Aris Scott and I am requesting to change my legal last name of Windom-Scott to the name I am known and referred.  I currently reside at 1079 Memory Lane, Swatara Township, Harrisburg, PA; and also lived at 2412 Chestnut Street, and 2903 Croyden, in Harriburg, PA. My transcripts as well as my diploma read "Aris K. Scott."  As I was being recruited this past school year my letters ans well as requests to visit colleges addressed me as "Aris Scott."  I am asking that I be granted my wish to make the name I am known and love by as my legal name,
