High speed internet - please put street name in comments

We would like for Fidelity Commuinications to expand their high speed internet options to the Salem/Avilla area where they currently do not have services. Please put your street name in the comments. 

I talked to Fidelity this week and they are doing a cost analysis to bring faster internet to our area. I want to have this list of signers ready for them. They need addresses and while I thought this petition would provide that to me, it does not. I hate to ask this, but could you fill out this 2 question form along with your address and optional comments?
