need approval to transfer court mandated community service

On May 1, 2013  I was admitted to the Providence Willamette Falls Medical Center Emergency room. I  received an extensive amount of blood labs, x- ray, ekg,mri, and cat scan tests. I was then admitted and then released on May 3, 2013. My diagnoses was cirrhosis of the liver, and gastoritis(gall stones) no blockage yet but will need soon eventually. Prior of these diagnosis, I was already diagnosed with Pitting edema, and stage2 hypertension., which should be noted by the prescribing doctor
