Force Medical Insurance companies to tell the truth

Several medical insurance companies (almost all) who offer PPO service (the top rated and most expensive medical service), and then when the service is used, restrictions and delays appear, or restrictions on use and access to drugs are imposed, without any medical review, and no prior warning of these restrictions.
Example (1)
Prescription antacid (normally prescribed once per day) was denied when prescribed twice per day by a qualified and licensed doctor.
Example (2)
Patient suspected with possible breast bone healing problems was denied cat scan because the insurance company thought that an standard Xray should be enough, even after being told that the xray was fogged by surgical wires that had been implanted 10 years before. A cat scan at an emergency visit later showed that the breast bone was almost completely open with only 2 wires holding it closed.
The are examples where PPO service has been reduced to the same level as HMO, but with much higher premiums. These companies are perpetrating a fraud on the public by charging for a service that is being deliberately reduced to a much cheaper service.
