Name Chang Petition

I am no longer married to Evans and would like to drop it from my name and all leagal documents the name is on. At the time I filed for  divorce he contested it that was one of the things I wanted in the divorce was to go back to Woods. The next year he filed for divorce and did not contact me. So I am asking that Evans is drop from my name.
