Demand the “Refusal to Vaccinate” form be eliminated

We the undersigned demand that the "refusal to vaccinate" form by the AAP be removed from all hospitals and pediatric clinics. Furthermore, we demand that all previously signed "refusal to vaccinate" forms signed before 5/6/2019 be rendered null and void. Parents/patients have the right to refuse any medical procedure they deem unnecessary or unsafe and should not be coerced into signing by threat of CPS or any other means.
