long term disability denial

I would like to appeal your decision on denying my Long Term Disability claim.  I realize that I did'nt want to file the claim but I really thought I would be back to work in a few weeks. 
Toyota has sent me a second letter to file for Long Term Disability in which I did because it now has been since 5/11/2011.  I should have filed with you earlier but I thought I was saving everybody time.

Please reconsider my case.
