Patient right to review their medical charts

People have the right to review their medical records. But when a physician puts things in their notes that are never discussed with the patient, the patient's healthcare is compromised. Physicians are randomly noting they make patients aware that they should have further testing and the words are never spoken to the patient. Be sure what is in your file is accurate. You have the right! I would suggest a physician should provide the notes to the patient at the end of each visit or, if unavailable, mailed to the patient upon completion of documentation.

I have had a serious, undiagnosed illness for over two years. Each physician has been no help to me.  Once I decided to take my health into my own hands, I requested all medical records from all physicians I have seen regarding this disease.  To my surprise, several of the physicians noted that they suggested I see other physicians or continue with further testing that they NEVER discussed with me.  I could have had a diagnosis and been treated a lot sooner if the physicians communicated better with me.
